Waiver of Injury Liability: 1, the undersigned, do hereby voluntarily submit my application for attendance and participation in the Combative Games League (CGL) and acknowledge that I am of sound mind and body, and am mentally and physically capable of participating in this CGL event, which is a rigorous, physical, and competitive event presented by Zulfi Ahmed, Bushi Ban International Llc and CGL. I hereby assume full responsibility for any/all damages, injuries that may occur, and any lost or stolen items that I may sustain/incur, if any. while attending or participating in this CGL event. I hereby waive all claims against the promoters, sponsors, organizers, management, officials, and volunteers. I also waive all claims of the same above stated nature against the promoters, sponsors, organizers, management, officials, and volunteers for the staff and management of the facilitates where the CGL is being held, individually or otherwise. I fully understand that any medical treatment given to me will be a first aid type treatment only. I consent that any pictures or videos taken of me in connection with my participation in the CGL can be used for publicity, promotion, internet or television showing. I waive compensation in regard thereto. IF UNDER 18 YEARS OF AGE, THIS RELEASE AND CONSENT MUST ALSO BE SIGNED BY A PARENT OR LEGAL GUARDIAN. The Tournament promoter holds the right of admission and can refuse attendance or participation to any applicant.
