PLEASE PUT THE DESCRIPTION OF YOUR DIVISIONS

LIABILITY WAIVER: I the undersigned, I in the consideration of the acceptance of entry and admittance into the above named tournament, hereinafter referred to as the "TOURNAMENT" do hereby release and forever discharge the producers, directors and/or competition in said TOURNAMENT. I assume all responsibility for my entry in the TOURNAMENT and do hereby state that I am physically able to compete in this athletic endeavor and have not been heretofore advised by a physician and/or medical authority against in said TOURNAMENT by instructor who is a Black Belt in the martial arts. any medical assistance that I am receiving will be of a first aid treatment type only. I also grant the ISKF ( International Sports Karate Federation ) permission to use any picture taken of me in said publication.

Executive 35 and up.

2) NO CHECKS OR CREDIT CARDS WILL BE ACCEPTED.

1) Some of the weight and (age) divisions may be combined or even further broken down if needed.
2) Youth/Teen/Adult will be closely matched according to age, weight and experience at the time of event.
